Change Application - This form is for changing your business address, changing your home address, replace a lost or damage license or an individual name change.
2010 Committee Request Form - Use this form to select the committees you are interested in participating in for the 2011 calendar year. Form must be turned in no later than October 31, 2010..

Continuing Education Compliance Form - This form must be filled out and submitted to the State of Ohio after you have completed the entire 30 hours of Continuing Education that is required.
Land/Lot Input Forms - This form is used in conjunction with firelandsmls.com for all Land/Lot listings.
License Renewal Form - The renewal applicaiton/education compliance form will not be accepted earlier than 60 days before your due date. If this is your first birthday since becoming licensed, your renewal is due, but not your 30 hours of continuing education.

Residential Input Forms - This form is used in conjunction with firelandsmls.com for all residential listings.
Scholarship Application - A letter of recommendation from your sponsoring Broker MUST accompany this application. You will receive notice of approval or denial within 30 days. Upon approval, after showing proof of successfully obtaining your R.E. license $250.00* will be applied to your FAR/MLS membership fees. *Subject to availability of funds.
Voluntary Hold Application -A licensee whose license is active or inactive may request that the license be put in voluntary hold status. Prior to a licensee's triennial renewal deadline, the licensee may put the license in voluntary hold status for up to 12 months. The licensee's renewal fee and proof of continuing education would be due at the time of reactivation. If the licensee does not apply to reactive the license in voluntary hold status during the subsequent twelve-month period or does not fulfill the renewal and/or continuing education requirements in that time, the license will be automatically resigned.
A resigned status is a permanent status. Once a license is resigned, it cannot be reactivated. A new license must be obtained in accordance with the requirements specified in Ohio Revised Code 4735.07 or 4735.09, as applicable.
